If you were registered with the Physiotherapy Board in the past but your registration has since been cancelled, you can apply to restore your registration at any time by submitting a *Restoration to the Register application.
You will be required to provide certified copies of the following documents in your application:
- Proof of Identity
- Name Change documents [if applicable]
If you are unsure of the current status of your registration, please contact our Registration Team before applying.
Please note, once you have submitted an application and your registration has been restored, you will then need to apply for an *Annual Practising Certificate (APC) before you can return to practising as a physiotherapist in Aotearoa | New Zealand.
